A Note from the CYS Nurse

Please keep in mind that if your child has any medical needs, additional documents and steps may be required that may extend your registration process. If your child has any change to their medical history, a new physical, signed and dated by a physician, is needed with a notation of the change.

Example: Child no longer required Albuterol per a healthcare professional, the healthcare professional can note the clearance of the diagnosis and treatment/requirement of medication in the History/Physical section of the HASP.

When in doubt, reach out! Our expert staff is here to help!
